## Key facts

- In a preprint, adding a GLP-1 receptor agonist to statin therapy was associated with 52.1% lower all-cause mortality over 3 years in ASCVD patients with obesity. (self-reported) [cites: 1794026835285607200]
- In a preprint, US adults in cardiovascular-kidney-metabolic stages 3-4 are 15.8% of the adult population but account for 76% of attributable years of life lost. (self-reported) [cites: 1794022874368594255]
- In a preprint, people with biomarker-defined stage B heart failure had a 15.2% adjusted 10-year mortality risk, close to 14.2% for overt stage C heart failure. (self-reported) [cites: 1793995107599437938]
- In a preprint, routine blood-count marker RDW was independently associated with growth of the dilated ascending aorta, with an odds ratio of 1.071. (self-reported) [cites: 1794022874368594254]

## The drug result

In a preprint posted this week, adding a GLP-1 receptor agonist to statin therapy was associated with substantially better cardiovascular outcomes in people with atherosclerotic cardiovascular disease and obesity. The study matched 119,911 pairs of patients from the TriNetX network and followed them for 3 years. All-cause mortality was 4.0% in the group that added the GLP-1 drug, against 7.5% on statin therapy alone, a 52.1% lower mortality risk. Myocardial infarction risk was 23.8% lower and ischemic stroke risk 32.9% lower. [cites: 1794026835285607200]

The result held up across subgroups, including people with a body mass index of 35 or higher, and across multiple sensitivity analyses. This is observational evidence from electronic health records, not a randomised trial, so it shows an association rather than proving the drug caused the difference. But the size of the cohort and the consistency of the signal make it the kind of finding that changes what clinicians ask about at the next visit: not whether the patient is on a statin, but what is on top of it. [cites: 1794026835285607200]

## Where the burden sits

A second preprint explains why that question matters now. Analysing nine cycles of US national health survey data from 2005 to 2023, researchers found that cardiovascular-kidney-metabolic syndrome burden is heavily concentrated in its late stages. People in stages 3 and 4 make up 15.8% of the adult population but account for 76% of attributable years of life lost. Mortality risk rises steeply with stage: the adjusted hazard ratio is 1.39 at stage 2 and 3.22 at stage 4, while stage 1 sits at 0.99, statistically indistinguishable from stage 0. The population is drifting in the wrong direction for prevention. Between 2006 and 2016, the share of adults at stage 0 fell by 2.3% a year while stage 1 rose by 3.1% a year. Smoking and physical inactivity account for 30.5% of the total burden. The authors argue that allocating resources by disability-adjusted life years systematically underfunds early-stage prevention, because the people at stage 1 look healthy on mortality statistics while they are quietly accumulating the risk that will dominate the burden later. [cites: 1794022874368594255]

## Cheaper signals, earlier stages

Two more preprints show how early the risk can be seen, and how cheaply. One studied 3,262 patients with serial CT scans and found that routine blood-count markers of inflammation track growth of the dilated ascending aorta. Red cell distribution width was independently associated with growth, with an odds ratio of 1.071 per unit, and in patients whose aorta had already reached the guideline threshold of 40 millimetres, all three markers studied were associated with interval growth. In aortas below 40 millimetres there was no association at all, which suggests the signal marks progression once dilation is established, not the start of it. [cites: 1794022874368594254]

The other preprint re-staged heart failure using a blood biomarker. In 11,477 US adults, people with stage B heart failure, defined by elevated NT-proBNP without self-reported heart failure, had a mortality risk nearly three times that of people with no risk factors, and statistically indistinguishable from overt stage C heart failure. Adjusted 10-year mortality was 15.2% for stage B against 14.2% for stage C. The catch is that adding the biomarker stage to conventional risk models barely improved prognostic discrimination, with a change in C-statistic of just 0.002. The test finds the high-risk people; it does not much improve the ranking of everyone else. [cites: 1793995107599437938]

## The gap between seeing and acting

Put together, the four preprints describe a system that can see risk earlier than it acts on it. A cheap blood count tracks a growing aorta. A single biomarker identifies people whose mortality risk already matches overt heart failure. A widely used drug class, added to a statin, is associated with halved mortality in a large matched cohort. And the national data say the burden is concentrated exactly where prevention arrives too late. The open question is not whether the signals exist. It is whether care reorganises around the early stages, where the risk is measurable and the mortality statistics still look normal, before the patients reach the stages where three-quarters of the lost years already sit. [cites: 1794022874368594255, 1793995107599437938, 1794026835285607200]

## What is still unresolved

- The GLP-1 findings come from an observational matched cohort, not a randomised trial, and no trial registration was provided.
- The heart failure staging study relies on survey data from 1999-2004, which may not reflect current treatment.
- The aortic growth markers have no prospective validation, and the inflammatory mechanisms remain unclear.
- The burden analysis lacks granular data on specific treatments within each disease stage.

## Decision card

- **Useful for:** Clinicians and health planners weighing earlier cardiometabolic intervention; Researchers working on biomarker-based risk staging
- **Not for:** Patients deciding on their own treatment; these are preprints, not clinical guidance
- **Compare against:** Randomised trial evidence for GLP-1 drugs in cardiovascular disease; Current guideline thresholds for aortic surveillance and heart failure staging
