## Key facts

- MedGemma 1.5 4B with structured radiologist descriptors is non-inferior to EU-TIRADS for thyroid nodule malignancy risk stratification, in a preprint. (self-reported) [cites: 1792870824598882115]
- HERA predicts HPV/p16 status in vulvar squamous cell carcinoma with an AUC of 0.84 in independent testing of 286 patients, in a preprint. (self-reported) [cites: 1792870824598839865]
- Synthetic e-prime velocity derived from 12-lead ECGs independently predicts all-cause mortality after acute myocardial infarction after adjusting for GRACE score, in a preprint. (self-reported) [cites: 1792859290845761422]
- Pairing decision-only medical AI with language models cuts framing-induced decision volatility by about 81% in development and 42-48% in held-out reports, in a preprint. (self-reported) [cites: 1792860587961261744]
- MedGemma's raw probability outputs are miscalibrated (ECE 0.117) and require Platt scaling for clinical use, in a preprint. (self-reported) [cites: 1792870824598882115]

## Thyroid nodules: matching the standard, cutting false positives

In a preprint from a Tunisian multicenter diagnostic accuracy study, a fine-tuned medical vision-language model, MedGemma 1.5 4B, combined with structured radiologist descriptors — the configuration called Config D — is non-inferior to EU-TIRADS for thyroid nodule malignancy risk stratification at ultrasound. Non-inferiority is the right bar here: the model is not being asked to beat the established scoring system, but to match it while changing something else about the workflow. [cites: 1792870824598882115]

The change it offers is fewer false alarms. Config D reclassified 72 of 205 benign nodules rated EU-TIRADS 4 as negative, reducing false positives. For patients, that is the difference between further workup and watchful waiting, and it is the kind of concrete benefit that justifies testing a model against a standard rather than against other models. The preprint is equally direct about the model's weakness: raw probability outputs from the fine-tuned model are miscalibrated, with an expected calibration error of 0.117, and require Platt scaling for clinical use. A model can rank cases correctly while still stating its confidence badly, and the authors say so plainly. [cites: 1792870824598882115]

## Cancer slides: molecular subtypes from histopathology

A second diagnostics preprint applies deep learning to histopathology slides in vulvar squamous cell carcinoma. The model HERA achieved an area under the curve of 0.84, with a 95 percent confidence interval of 0.75 to 0.91, for predicting HPV/p16 status in independent testing of 286 patients. Predicting a molecular marker from a routine slide, rather than from a separate molecular test, is the practical prize: the information is already sitting in the pathology workflow. The companion model, HERMES, goes further, predicting molecular subtypes whose prognostic hierarchy the preprint reproduces: five-year recurrence rates of 14.0 percent for HPV-associated disease, 31.1 percent for HPV-independent and p53-wildtype disease, and 43.3 percent for HPV-independent and p53-abnormal disease. But the preprint also reports the limit. HERMES demonstrated limited separation of the two HPV-independent subtypes, with areas under the curve of 0.68 for the p53-abnormal subtype and 0.63 for the p53-wildtype subtype. The model separates the broad groups well and the hardest pair poorly, and any clinical use would have to live with that boundary. [cites: 1792870824598839865]

## Heart signals: mortality risk hidden in an ECG

A third preprint finds prognostic signal in a routine test. Synthetic e-prime velocity derived from 12-lead ECGs is an independent predictor of all-cause mortality after acute myocardial infarction, after adjusting for GRACE score, the established risk tool. MAPSE derived from 12-lead ECGs is a significant predictor of myocardial-infarction-specific mortality in the general population, with a subdistribution hazard ratio of 0.49. The point of deriving left-ventricular mechanics from an ECG is reach: ECGs are far more available than the imaging that normally measures these quantities. [cites: 1792859290845761422]

The preprint adds that synthetic left-ventricular mechanical parameters provide incremental prognostic value over measured left-ventricular ejection fraction and over AI-ECG-predicted ejection fraction. Incremental value is the honest test for a new biomarker: it must add information beyond what clinicians already have, not merely repeat it in a new format. Here, the ECG-derived mechanics clear that bar in the preprint's analysis. [cites: 1792859290845761422]

## Triage: pairing models to steady the decision

A fourth preprint addresses a different failure mode: framing. Decision-only models and language models exhibit significant framing sensitivity, with decision changes of 13.4 percent and 14.0 percent respectively when neutral wording is replaced by labels or pressure cues. A triage tool that changes its answer because of how a case is worded, rather than what the case contains, is a safety problem even if its average accuracy looks acceptable. [cites: 1792860587961261744]

Pairing decision-only models with language models reduces framing-induced decision volatility by approximately 81 percent in development and 42 to 48 percent in held-out reports, while the paired workflow maintains triage accuracy between 81 and 82 percent at a cost of 0.77 US dollars per 1,000 entries. The held-out figure is the one to weight: the benefit shrinks outside development data, but it does not vanish, and the cost figure gives buyers a concrete denominator. Across all four preprints, the pattern is the same — models judged against clinical standards, with their calibration, separation, and framing limits reported alongside their wins. [cites: 1792860587961261744]

## What is still unresolved

- Whether MedGemma's calibration holds after Platt scaling in prospective clinical use.
- Whether HERMES subtype separation improves for the two HPV-independent subtypes.
- Whether paired triage workflows hold their accuracy and cost outside the studied reports.

## Decision card

- **Useful for:** Clinicians and health-system buyers evaluating diagnostic AI against existing standards.; Developers building clinical models who need to report calibration and framing limits.
- **Not for:** Replacing EU-TIRADS, GRACE, or pathology review on the basis of preprints alone.; Patients making individual treatment decisions.
- **Compare against:** EU-TIRADS scoring, GRACE risk adjustment, and measured ejection fraction.; Single-model triage without language-model pairing.
